YOUTH FORMS
To register for a trip or event, please fill out an Event Registration form (if you are a new student or this is your first event of the calendar year, please also fill out the appropriate Medical Release form). There needs to be a separate Event Registration form for each event. Submit any forms along with deposits or payment to the Youth or Church office or send with your student to youth group/small group meetings.
Scholarships: We never want money to be a factor in a student deciding to attend a retreat or trip! For information on scholarships, please email Adam Miller at amiller@firstpreshbg.org or download the Scholarship Form below and submit to the Youth/Church office. All Scholarship forms are kept confidential.
